To apply to Adopt a Spot, please fill out the form below with the following instructions. Note: You will need the ID number, park name or location of the spot(s) you'd like to adopt, which are listed on the Adopt a Spot map.
- Enter your home address and click "Confirm Location." If you live outside of Plymouth, please use the address for Plymouth City Hall, 3400 Plymouth Blvd., and click "Confirm Location."
- Select "No Photo."
- Leave Description text box blank.
- Choose all spots you wish to adopt and fill out the remaining fields to complete the application.
- Click "Confirm Details" when complete.
- To submit the request, please select "Submit as Yourself" – your name and information will be available to city staff, but will not be visible to the public.
After you have confirmed details, you will receive a follow-up email acknowledging your Adopt a Spot request (the Adopt a Spot form is powered by a tool called SeeClickFix, which will ask applicants to set up an account).
